Job Description

1) Provides advocacy and support for Deaf, DeafBlind and Hard of Hearing (DDBHH) community members who
have cancer and may have challenges advocating for themselves with providers and other healthcare
professionals. Most meetings with clients take place in the CHS office. However, usually due to clients’
transportation limitations, some meetings may occur at agreed-upon locations in the community.
2) Provides culturally and linguistically appropriate breast health education.
3) Assists clients with problem-solving barriers to address health conditions by identifying, connecting to and
navigating needed community and medical system services.
4) Assists clients with completing necessary forms.
5) Establish a trusting relationship and continuous candid communication with clients and client’s family
support systems.
6) Advocates for individual and community health equity.
7) Assists in referring and connecting to appropriate preventive care services.
8) Collaborate with clients and/or family members, providers, clinic staff and other relevant agencies to assure
appropriateness of services that meet member needs and ensure desired outcomes.
9) Provide support, follow up and education.
10) Educates clients about their rights and responsibilities in order to encourage self-advocacy. When
necessary, accompanies clients to appointments and other service providers.
11) Creates and implement health educational materials that are accessible to DDBHH people.
12) Creates resource directories including local and national information, treatment centers, websites, Facebook
pages, etc.
13) Provides information and education at community events, speaking engagements, health fairs, community
gatherings and other opportunities.

14) Implements data collection procedures to document service delivery and demographic information about clients of the patient navigation program.
15) Responsible for reporting according to state and private funding and assists with grant and contract writing as needed.
16) Performs other related duties as assigned.

Requirements

Fluency in American Sign Language, knowledge of Deaf culture required.
▪ Bachelor’s degree in social work or related field required.
▪ Must possess a valid driver’s license and have access to an operational vehicle to transport self to various locations throughout the metro Chicago area and collar counties.

THIS IS A FULL-TIME POSITION FOR A TERM OF ONE YEAR (the continuation of program is contingent on renewal of funding).
