Job Description | Greater Nashua Mental Health is looking for a full-time Case Manager to join our Deaf & Hard of Hearing Services Team! The Case Manager is responsible for direct support, education, and outreach to a caseload of adults diagnosed with a severe mental illness, while managing cases. Meets with individuals statewide within their natural environments, provides community based services to support the conditions for mental health treatment, crisis intervention, and education; assists in removing barriers to improving functioning so that optimal recovery can be obtained.
